New Patients section contains all the paperwork that you would normally expect to receive and fill out upon your first visit to our clinic.  These are provided here for patient convenience, allowing you to fill out the new patient forms in the comfort and convenience of your own home. 

Download these pages [be sure to get them all], print them to your printer, fill them out, then bring them with you on your first visit to our clinic, along with a list of your current doctor prescribed medications, copies of your current insurance cards and a photo id. Each page is a separate download.  All pages can be viewed prior to printing, using Adobe Acrobat Reader. 

PLEASE DO NOT MAIL THESE FORMS BACK TO US, JUST BRING THEM WITH YOU ON THE DAY OF YOUR APPOINTMENT.

Handouts section is provided as a patient or prospective patient educational tool.  These are educational brochures that we have developed regarding various ophthalmic topics, that hopefully will provide you with additional knowledge of certain ophthalmic conditions and procedures.  You may download as many as you like.  You may view them on your computer using Adobe Acrobat Reader or you may print them out on your printer.
